About Rooted Together Village
Rooted Together Village is an aging-in-community membership service culturally tailored to residents age 50+ living in the North Nashville/Bordeaux footprint. Membership provides a Community Resource Navigator to each Village member to support their continued health and well-being with seamless connections to resources.
Rooted Together Village is an affiliated entity of Community ConneXor, a technology-enabled community care hub development and management company, supporting existing community based organizations to improve how they engage with and connect their clients to human and social service resources to decrease access and navigation challenges. The Community ConneXor community care hub model is among the fifty-eight (58) organizations from across the nation, and the only Tennessee entity, selected by the Administration of Community Living to participate in its first community care hub national learning community.

Increase and enhance awareness, navigation, and utilization of community resources to improve overall health and well-being.
What is a Community Resource Navigator?
A Community Resource Navigator is a trained individual to help Village members and their families to access and navigate basic need services and other community resources available. Village navigators are trained utilizing a Community Health Workers curriculum to ensure culturally tailored assistance ranging from identifying to coordinating basic need services.
How does Rooted Together Village – Aging in Community work?
Members can work with a Community Resource Navigator to get assistance as needed by expressing a need to a Community Resource Navigator at monthly sponsored events, calling or emailing. Additionally, each month the member’s assigned Community Resource Navigator will initiate an Aging in Community Check-In call. RTV maintains a directory of preferred providers and local businesses offering a variety of professional services (and any applicable discounts). Navigators will not only help identify a resource but stay engaged as needed to ensure the member’s need is met.

- Initial Aging in Community Screening to identify potential resource needs.
- Access to all RTV sponsored activities.
- Access to resource navigation services provided by our trained and vetted RTV Community Resource Navigators.
- Access to RTV preferred providers.
- RTV Calendar of Activities and newsletter delivered to you to keep you up to date on Village offerings.
- Free, fresh produce boxes from mid-September through mid-November 2023.
- Other food security enhancing programs.
- Monthly Aging In Community Check-In Calls.

POSITION OPENING: Community Resource Navigator

The Community Resource Navigator (CRN) position is a part-time (up to 20 hours per week), non-clinician role which functions in the coordination and management of non-medical supportive services and resource navigation for Community ConneXor’s Rooted Together Village – Aging in Community (Village).
National Council on Aging (NCOA) – Rooted Together Village received funding from NCOA to support its proposal entitled “No Wrong Door: Vaccine Uptake and More”. The “More” in “No Wrong Door: Vaccine Uptake and More” is focused on basic needs services older and disabled adults also required for supporting their health and well-being in the community. The NCOA grant period is April 2023 through April 2024.

The Village is working in collaboration with Happy Hearts Smile, Inc., Vaccine For All, The Nashville Food Project, Second Harvest Food Bank of Middle Tennessee, Black Mental Health Village, and Urban Housing Solutions, Inc. And, Rooted Together Village is in partnership with CommunityConnexor and National Council on Aging.
